Publication release form – Fill out to allow the Collaborative School of the Arts to take videos and pictures of your student

Emergency contact form– Fill out so the Collaborative School of the Arts has all necessary information for emergency situations

Student release form – Fill out so the Collaborative School of the Arts can release the student, and have them picked up by an appropriate parent or guardian.

Lunch release form – Fill out to allow your child to leave the classroom for lunch

ONLY FOR TECHNICAL THEATRE CLASSES

Parent consent form – Fill out to allow your child to use various power tools and equipment

Student safety protocols – Have your child fill out to agree to our safety measures in regards to power tools and equipment

ONLY FOR FILM/MEDIA CLASSES

Location filming release form – Fill out to allow your child to be escorted to areas in and around the Proctors campus for filming with their group.
